Set the Form Title

• Enter a Title that describes the purpose of the form.

Add Fields and Select Field Type

• Click Add Field to create a new input field.

• Enter field label

• Choose a field type from the following options:

  • Short Answer – Single-line text input.

  • Paragraph – Multi-line text input.

  • Dropdown – Users select one option from a list.

  • Multiple Choices – Users select one option from multiple choices.

  • Checkboxes – Users can select multiple options.

  • Date – Users pick a date.

  • Slack User – Select a user from Slack.

• Turn on Required toggle if field is mandatory.

Add Option for Selectable Fields

If the field type is Dropdown, Multiple Choices, or Checkboxes, provide the available options.
